| The Cancer Survivor Wall has affixed named plates of existing and past cancer survivors. Donors and supporters who also believe in keeping Mizpah grounds going may purchase nameplates in memory of loved ones. Scroll to the bottom of the page for a nameplate form. |
| The Engraved Nameplate affixed on the Cancer Survivor Wall helps support the existence of Mizpah’s grounds for years to come. Please note the various check of circles to use. Simply give us the name of the individual that will correspond with the check of section. For example if the individual is a: Past & existing cancer survivor: The name and address of the past or existing cancer survivor will be engraved on the nameplate. Please note: If the individual is deceased give us the date (R.I.P.). Supporter: Here your name and address will be engraved on the nameplate as a supporter. In Memory of: The name and address of the individual in memory of you want us to engrave on the nameplate. ie: Mr. & Mrs. |
