“ to help another”.
How to Make a Walking Team using the
Candle Sign-up Sheet
Somebody takes charge and forms a team. Your team may be made up of family members, brothers & sisters, a group of co-workers from a department store, a group of card players, golfers, club members, or friends, etc. Your team may consist of two or as many as you want it to be. The goal is for each team to raise a minimum of $100.00. The team will walk in honor of a cancer survivor (past or present) which they have collected for. For each $5.00 collected, a candle will be place in a white paper bag along with the name of the cancer survivor and the person’s name that donated in honor of the cancer survivor and placed around Mizpah’s Pond. The candle will be lit-up at dusk and will stay lit all night in memory of that individual while people casually walk around the pond. What a marvelous site to see. If you collect over $100.00 in honor of the individual; his/her name will be engraved on a nameplate and be permanently placed on the Cancer Survivor Wall.
You may participate in this walk-a-thon should you live too far away by filing out the Cancer Survivor Wall form along with your $100.00 donation to the address mentioned. Give us the name of the individual and check the past & existing cancer survivor box. (Give us the R.I.P. date if you need to). His/her name will be placed on the Cancer Survivor Wall.
There are also sections on the wall for individuals who are supporters of this project.
Simply fill out the form with your name, phone number and check the supporter box. If you donate in memory of someone, simply enter the name and check the in memory of box. Upon receiving your donation a nameplate will be engraved and attached to the Cancer Survivor Wall.
